The Community Wellbeing Connectors provide an integral role within the Community Mental Health Transformation within Leeds. The post holder will support people with Severe Mental Illness to achieve improved mental health and wellbeing, increase independence and support their recovery. They will work as part of a Multi-Disciplinary Team within the HATCH Local Care Partnership.

The post holder will support a caseload of clients using a person-centred and trauma-informed approach. They will support clients to identify services within the local community that will support their recovery and will motivate and enable them to engage with these services.
